Apply to be a Rider

Follow These Steps to Submit Rider Application:

1. Download and Print Rider Application  CLICK HERE

2. Complete all pages.

3. Submit “Physicians Referral” form to doctor. Doctor must complete TEC’s form. We cannot accept a camp or sports form.

4. Return ALL completed pages to TEC:

a. Scan ALL pages and Email to gerrypagetec@gmail.com OR

b. Mail ALL pages to: 537 Northampton St,. Holyoke, MA 01040
